Our mission is to provide outstanding care to patients & healthy, sustainable work environment to our employees. We have created an attendance policy to support that mission. Each appointment is scheduled for only one patient. We do not overbook our providers. Appointment attendance is important to maintaining patient health as well as the health of the practice.
Cancellation List:
Greater than 3 cancelled / rescheduled visits in a 12 month period will move the patient onto our Cancellations List. Once placed on the list, patients will not be able to schedule an appointment in advance. Patients can be seen by requesting same day availability at the time they need to be seen. If there is same day availability, the patient will be placed in that appointment. Requests can be made by calling or texting the office. After successfully attending 3 same day appointments, patients can return to scheduling appointments in advance. Please note medication refills cannot be accommodated during this time and provider preference is not guaranteed.
No Shows / Same Day Cancellations:
Cancellation with less than 24 hours notice is considered a Same Day Cancellation. A missed appointment without notification is considered a No Show.
The first occurrence in a 12 month period is assessed a $50 fee. The second occurrence in a 12 month period is charged a $100 fee. The third occurrence may result in dismissal from the practice.
